Near infrared spectroscopy monitoring during carotid endarterectomy in elderly patients

چکیده
Materials and methods 193, consecutive patients, 101 males, mean age 72 years, underwent CEA in our Institution from September 2007 to December 2009. All operations were only performed by two surgeons and all patients were monitored with TCO during general anesthesia. TCO parameters were always registered in: base-line, anesthesia induction, carotid clamping and declamping, wake up and estubation. The relationship with TCO was described in terms of the decrease in percentage oxygenation. Standard endarterectomy was performed through a longitudinal arteriotomy. The majority of arteriotomies were sutured with a dacron patch. Primary suturing was done only when it was possible to use the eversion technique.

Near-Infrared Spectroscopy versus Transcranial Doppler-Based Monitoring in Carotid Endarterectomy
Background Proper monitoring of cerebral perfusion during carotid artery surgery is crucial for determining if a shunt is needed. We compared the safety and reliability of near-infrared spectroscopy (NIRS) with transcranial Doppler (TCD) for cerebral monitoring. Methods This single-center, retrospective review was conducted on patients who underwent carotid endarterectomy (CEA) using selectiv...
متن کاملMonitoring cerebral ischemia during carotid endarterectomy and stenting
Current therapy for carotid stenosis mainly includes carotid endarterectomy and endovascular stenting, which may incur procedure-related cerebral ischemia. Several methods have been employed for monitoring cerebral ischemia during surgery, such as awake neurocognitive assessment, electroencephalography, evoked potentials, transcranial Doppler, carotid stump pressure, and near infrared spectrosc...
متن کاملEvaluation of an intraoperative algorithm based on near-infrared refracted spectroscopy monitoring, in the intraoperative decision for shunt placement, in patients undergoing carotid endarterectomy.
BACKGROUND We evaluated whether the use of an intraoperative algorithm based on cerebral oximetry with near-infrared refracted spectroscopy (NIRS) monitoring, could aid in the intraoperative decision for shunt placement, in patients undergoing carotid endarterectomy (CEA). METHODS In this prospective, randomized, controlled study were included 253 patients who underwent CEA under general anes...
متن کاملAccuracy of cerebral monitoring in detecting cerebral ischemia during carotid endarterectomy: a comparison of transcranial Doppler sonography, near-infrared spectroscopy, stump pressure, and somatosensory evoked potentials.
BACKGROUND This study compares the accuracy of cerebral monitoring systems in detecting cerebral ischemia during carotid endarterectomy. METHODS The authors compared transcranial Doppler sonography (TCD), near-infrared spectroscopy (NIRS), stump pressure (SP) measurement, and somatosensory evoked potentials (SEP) in 48 patients undergoing carotid surgery during regional anesthesia. Cerebral i...
متن کاملIs Near-Infrared Spectroscopy a Reliable Method to Evaluate Clamping Ischemia during Carotid Surgery?
Guidelines do not include cerebral oximetry among monitoring for carotid endarterectomy (CEA). The purpose of this study was to evaluate the reliability of near-infrared spectroscopy (NIRS) in the detection of clamping ischemia and in the prevention of clamping-related neurologic deficits using, as a cutoff for shunting, a 20% regional cerebral oxygen saturation (rSO(2)) decrease if persistent ...
